Our work engages partnerships at the local, state, and national levels: Local We run the Camden Core Model, a nationally-recognized care intervention for individuals with complex health and social needs. Lessons from our interventions and citywide coalitions inform clinical innovations, partnerships, and organizational priorities. Local Initiatives State Our state-level advocacy work is based on our patients’ needs and insights from our regional partners. We convene healthcare and social services providers, health systems, payers, and healthcare consumers to advocate for policy change and better-coordinated care. State Initiatives National Our National Center for Complex Health and Social Needs works with stakeholders to build a field of complex care. We are strengthening the field by providing strategic guidance and technical assistance based on our work in Camden.
